The Tuition Reimbursement Application Form is for the employee to fill out in order for the employer to reimburse his/her tuition. The form states the following:

I understand that I must attain an A, B, or C grade in each course in order to be eligible for 100 percent refund of tuition only (does not include books, registration fees, etc.). D grades are reimbursed at 50 percent of the tuition. Also, there will not be any tuition reimbursement if I fail the course. Correspondence type courses qualify for reimbursement only when a like course is not available at an accredited institution.
